Our catalog is already available in many facilities, and your loved one can use it to select their frames. This guide is for you—the family member or friend—to help you confidently and correctly complete the order.


What if My Loved One Doesn't Have a Catalog?

 

If your loved one doesn't have access to our catalog, you can request one be sent to them. Please fill out our contact form with the following information:

  • Inmate's full name and ID number

  • Facility name and full mailing address




Step 1 (The Most Important): Check All Facility Rules

 

This is the most critical step. Every facility has its own set of specific rules. Before placing an order, you (or your loved one) must confirm these rules.

We are not responsible for orders rejected by a facility, so it's always best to check first.

Your "Must-Ask" Checklist for the Facility:

  • Vendor Approval: "Is 'Choice Eyewear' an approved vendor?"

  • Frame Materials: "Are metal frames/components allowed, or must they be all-plastic/acetate?"

  • Frame Colors/Types: "Are any colors, patterns, or lens types (like sunglasses or transitions) banned?"

  • Price Limits: "Is there a price limit on what can be sent?"


 

Step 2: How to Place the Catalog Order

 

Once your loved one has their catalog and a valid prescription from the facility's medical unit, there are three simple ways to place an order.

 

Option 1: Inmate Sends Order Form for You to Pay 

 

  1. Your loved one fills out the order form from the catalog with their prescription and frame choice.

  2. They mail the form to us and list you as the payer/contact person.

  3. Our team will contact you directly by phone or email to confirm the order details and collect payment.

 

Option 2: Inmate Sends Order & Payment Directly

 

Your loved one can complete the process themselves.

  1. They fill out the order form from the catalog.

  2. They mail the completed form to the address in the catalog, along with a payment from the institution (money order or check).

 

Option 3: You Order Online (The "Fast Pass")

 

This option is for family members who know exactly what their loved one wants.

  1. If you know the exact frame model from the catalog, you can order it directly on our website.

  2. At Checkout:

    • Add the inmate's ID number after their last name (e.g., "John Smith #12345").

    • Use the facility's full, correct address as the Shipping Address.
